JaxParks Summer Camp Voucher Information
Voucher applications for the JaxParks summer enrichment camps will be received and vouchers issued from noon to 6 p.m. on April 22 and 23 at the JaxParks offices, 851 N. Market St. downtown Jacksonville.
The number of vouchers is limited and will be issued on a first come, first served basis. Qualifying information determined by the Jacksonville Children's Commission is as follows:

Children must be: |
- currently in kindergarten through 15 years of age.
- a Duval County residents

AND either: |

Special Needs/Special Education:
- Form from a school with current date (school year).
- A current diagnosis report from Nemours or other medical/psychological evaluation verifying the child would require an IEP if enrolled in public schools.
- McKay Scholarship (letter or form with child's name and current date).

Family Income at or below 185% of the Federal Poverty Level:
- Free and Reduced Lunch formal letter or form form school for current year.
- A current copy of their Heroes Scholarship letter.
- Verification household family income does not exceed 185% of the Federal Poverty Level. (See chart below)
